  • Coor­di­nate and ensure on time deliv­ery of project deliv­er­ables includ­ing user require­ments, func­tional spec­i­fi­ca­tions, infra­struc­ture set-​up, project doc­u­men­ta­tions, etc.
  • Man­age and com­mu­ni­cate project activ­i­ties among ven­dors and stake­hold­ers of the projects.
  • Main­te­nance review of rou­tine main­te­nance task, sched­ule, per­for­mance mea­sure and analy­sis, inven­tory and asset tracking.

REQUIRE­MENTS:

  • Min­i­mum diploma holder.
  • Five to six years’ rel­e­vant work­ing expe­ri­ence in a non-​profit organ­i­sa­tion or cor­po­rate environment.
  • Good knowl­edge in MS Office and Server appli­ca­tions such as MS Exchange, NT server and SQL server and Web tech­nol­ogy will be an advantage.

RESPON­SI­BIL­I­TIES:

  • Con­duct needs assess­ment of cases referred for admis­sion, i.e res­i­den­tial place­ment, respite, DAC, etc.
  • Com­plete a social his­tory and psy­choso­cial assess­ment for each res­i­dent that iden­ti­fies social, emo­tional and psy­cho­log­i­cal needs.
  • Par­tic­i­pate in the devel­op­ment of a writ­ten, inter­dis­ci­pli­nary plan of care for each res­i­dent that iden­ti­fies the psy­choso­cial needs/​issues of the res­i­dents, the goals to be accom­plished and the appro­pri­ate social worker intervention.
  • Par­tic­i­pate in review­ing and set­ting poli­cies con­cern­ing res­i­dent care and qual­ity of life.
  • Develop, main­tain and utilise cur­rent com­mu­nity resources that are use­ful to res­i­dents and their families/​significant others.
  • Assist res­i­dents and their families/​significant oth­ers with attain­ing resources that fit their needs includ­ing the pro­vi­sion of finan­cial assis­tance when required.
  • Under­stand and meet all Gov­ern­ment require­ments for social ser­vice documentation.
  • Iden­tify com­mu­nity changes and oppor­tu­ni­ties such as leg­is­la­tion, reg­u­la­tions and pro­grammes that affect residents.
